For destroyed Demi Moore with G.I. Jane always comes to mind and doesn't really help the fact that Striptease was the year before that as well. She just disappeared.
Gigli was god awful. Didn't ruin Ben Affleck or JLo, although director Martin Brest on the other hand..
For most helped I believe it has to be either Al Pacino in Godfather or Bruce Willis in Die Hard.
